We want to empower you to make the best choices for you and your family which is why we offer on-line information and in-person events to help you understand your insurance coverage options. The benefits offered to eligible employees and dependents are exceptional and we hope these resources help you make the most of what is offered.
- You must complete an Enrollment Form and return it to Human Resources at hr@mcohio.org within 30 calendar days of your hire date (or part-time to full-time date)
- If you do not submit an enrollment form within 30 calendar days of your hire/transfer date, you will have no insurance coverage. The only way to enroll at a later date is if you experience a Qualified Life Event or during the next open enrollment period.
- You must provide dependent documentation in order to add dependents to your plans. See Dependent Eligibility.
- Use the checklist for the documentation to be submitted with your enrollment form.
- Refer to the Insurance Effective Dates for the date your insurance coverage will take effect.
- If you want to waive medical insurance coverage and receive a waiver credit, complete and return a Verification of Employer-Sponsored Health Coverage.
